For your convenience, we accept all major credit cards. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. We work hard to maximize your insurance benefits but please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to the actual procedure.

Platinum Membership Program
Platinum Membership Program is an annual reduced-fee savings plan for families and individuals that allows all Platinum Membership Program Members to receive quality dental services at greatly reduced prices. Unlike conventional insurance plans, with Platinum Membership Program there are no deductibles, no yearly maximums, and no waiting periods to begin treatment. Platinum Membership Program benefits coverage begins immediately on plan registration.
Benefits include:
- Free simple teeth cleanings (up to two per year)
- Free complete annual dental exam
- Free x-rays for complete annual exam
- Free fluoride applications (up to two per year)
- A 15% discount on all dental procedures
A Platinum Membership Program Membership is $499 for adults and $259 for children 12 years old and younger. All plan membership fees are payable in full at the time of registration and are non-refundable. Plan duration is for one year from the registration date. All Platinum Membership Program Membership fees are due and payable at the time of registration. Interest-free payment plans of 6 and 12 months’ duration are available on request with approved credit. Repayment duration is based on service totals. When a payment plan is used, your Platinum Membership Program Member Discount will be reduced to 10%. If a financing service such as CareCredit is used, the patient will no longer receive the 15% discount. Please notify our office 24 hours in advance, if you must change a scheduled appointment.
